If you are wanting to become a new patient at Good Samaritan Health Services, you will need to fill out a New Patient Appointment Request Form. Please be aware that appointments are not guaranteed, and appointment slots are very limited. Patients must reside in the Tulsa area, be uninsured, and not be established at another Tulsa community clinic. You will be notified of your appointment in 3-4 weeks by phone and/or text only if you are accepted as a new patient. If you do not receive notification, you will need to seek care elsewhere.
