Fazel Family Foundation, Inc.

Thank you to the Fazel Family Foundation for grant funds to purchase a one-year subscription for a software program called “Patient Assistance Program Tracker.” This program will give our nurses the tools they need to more readily assist patients in applying to pharmaceutical companies for medication assistance.

Many pharmaceutical companies offer expensive drugs at low or no cost to low-income patients through patient assistance programs. However, each company has its own application, these are often lengthy. Some of our patients are on multiple medications to manage their health conditions.

Having this program will increase our patients’ access to medications they need but cannot afford, and we are unable to provide.
